Physical Form & Required Information for UIL HS Band & Guard Participation

The UIL requires any student entering a new school-sponsored athletic group (including marching band & guard) to complete a physical before they are allowed to participate. All student physicals should be dated no earlier than May 1, 2026 to be eligible for the 2026-2027 school year. ALL FORMS ARE SUBMITTED ONLINE THROUGH RANK ONE. PLEASE DO NOT TURN THESE FORMS IN TO A DIRECTOR.

Presto Assistant Management

We use Presto Assistant as a data management and financial tracking tool for the band. You can log onto Presto www.presto-assistant.com at any time to see the status of your student’s financial account. Statements will be sent out monthly with your current balance.

Varsity Marching Band $250 

(ONLY varsity band and color guard students pay this fee.)

We kindly ask that you hold off on paying this fee until your child has been officially accepted into the Varsity Marching Band. This fee is due by August 1st.

* The duffle bag logo is like last year’s. The logo pictured in the shop has a smaller logo than what will be printed.

** There are no extra items in the band hall. If your student loses their required “Spirit Gear,” it will need to be repurchased through this site.

** If your 10th-12th grade student has the following items from last year: Red Melissa Band T-Shirt, Black Melissa Band Shorts, the duffle bag and the water jug, you do not need to order these items for your student. Please note that all items on this list are required this year.

Friday Night Game Food Option $100

For $100, students can have a meal provided to them for all 10 football games. 

Please understand:

• Students won't have the option of leaving school before home games. 

• Uber Eats & other food delivery services won't be allowed to students. 

• Parents must provide dinner for students every football game. 

• The food option is highly recommended, as this is the easiest option for all families. Game day can be very busy!

Melissa ISD School Owned Instrument Rental Fee $125

ONLY for students who play school-owned instruments.

Below are school owned instruments:

Oboe, Bassoon, Bass Clarinet, Tenor Saxophone, Bari Saxophone, French Horn, Bass Trombone, Euphonium, Tuba, and Percussion.

No need to repurchase items your student already owns. Your student will definitely need to order additional flip folder pages and if they have lost it, a lyre for their instrument.

Upgrade your student’s “beginner” instrument to an advanced model

It is recommended that you keep your student’s “beginner” instrument for marching band use. We recommend that students not continue to use their "beginner" instruments past the 8th grade, unless an advanced model was purchased initially.  Better quality instruments enable all students to learn more quickly and with greater ease.
